On Lok Membership Information
Your On Lok Lifeways care team is available to discuss any questions or concerns that you may have. Perhaps, it is as simple as needing a new ID card. We will make sure that you get a new one. Or, maybe you want to talk about the care that you or a loved one is receiving. We will provide you with all the important information that you require. No matter the reason for your inquiry, your opinion matters to us.
The safety and comfort of the On Lok Lifeways PACE participants is our upmost concern. We care about our seniors, and we want to make sure they are well taken care of and receiving quality care while also maintaining the independence that they value.
